Florida Immunization Requirements
Inspire Learning Academy follows all Florida Department of Health immunization requirements for private schools. Students must submit the required documentation before attending classes.
Every student must provide ONE of the following before the first day of attendance:
- Florida Certificate of Immunization (DH Form 680)
- Florida Religious Exemption (DH Form 681)
- Permanent or Temporary Medical Exemption documented on DH Form 680
Kindergarten
Students entering Kindergarten must provide a current Florida DH Form 680 documenting all required school-entry immunizations or an approved exemption before attending school.
3rd Grade
Parents are encouraged to review their child's immunization records during 3rd grade to ensure all required vaccines remain properly documented on the student's Florida DH Form 680.
6th Grade
Families should review immunization records before the end of 6th grade to prepare for the required Tdap booster needed for 7th grade enrollment.
7th Grade
Florida requires one dose of the Tdap vaccine for all students entering, attending, or transferring into 7th grade unless a valid exemption has been approved. An updated DH Form 680 must be submitted. :contentReference[oaicite:1]{index=1}

Schedule a Physical

- Temporary medical exemptions must be renewed before expiration.
- Permanent medical exemptions must be documented by an authorized healthcare provider.
- Students transferring from another state may need their immunization records converted to Florida DH Form 680.
- Immunization records become part of the student's permanent educational record.
- Parents should notify the school whenever updated immunization records are received.
